Withdraw Forms

To begin the withdrawal process, please complete the form at the link below.  Your student's withdrawal will be complete when Falcon High School and your student's new school exchange attendance confirmation documentation.

 

If you are electing to homeschool your student, please request a Notice of Intent to Homeschool form from the registrar.
